What Is Health Care Fraud?

Photo of a magnifying glassHealth care fraud is a crime that affects everyone. Reducing health care fraud and abuse can help keep down costs.

The most common examples of health care fraud include billing for services, procedures or supplies that were not provided and the intentional misrepresentation of any of the following to manipulate the benefits payable:

  • The nature of services, procedures or supplies provided
  • The dates on which the services or treatments were rendered
  • The medical record of service treatment provided
  • The condition treated or diagnosis made
  • The charges or reimbursement for services, procedures or supplies provided
  • The identity of the provider or the recipient of services, procedures or supplies

You can take the following simple steps to help reduce fraud and abuse:

  • Examine medical bills closely to verify services.
  • Ask for an explanation or for clarification of any unusual or questionable services.
  • Call Paramount’s Member Service Department for confidential handling at 1-419-887-2525 or toll-free 1-800-462-3589. TTY services for the hearing-impaired are available at 1-800-740-5670. You also may contact the ProMedica Health System Compliance Hotline at 1-800-807-2693 for confidential investigation.
