We developed the Paramount Preventive Health Care Guidelines to help you play an active role in your own and your family’s health care. You can use the information in these tables to schedule the services you need, when you need them.

Regular visits with your PCP -- even if you're feeling good -- may help prevent serious health problems that could bring you to the doctor’s of?ace, hospital or emergency room in the future. Finally, keep in mind that the information in these tables is appropriate for those with average risk for the conditions named. If you or a family member is at high or above-average risk, or you have a chronic health condition such as diabetes or high blood pressure, you'll need to talk with your doctor to develop a plan that meets your personal health care needs.

Safeguards Protect Your Confidentiality

Paramount Elite maintains your documented personal health information, in writing or verbally, in the strictest confidence. Every Paramount employee is required to follow Paramount’s confidentiality policy as a condition of employment and signs an annual certification on confidentiality procedures.

Photo of a mature manWhen you are enrolled in Paramount Elite, you give consent for routine use of identifiable information needed for conducting quality assessment, claims processing, utilization review, fraud detection and oversight review.

Except when required by law, we will not release any identifiable personal health information to a third party without first giving you the opportunity to approve or deny the request.

Any organization performing quality reviews for accreditation purposes or regulatory agencies also is required to maintain confidentiality.

We do not maintain medical records. We only maintain administrative records related to your benefit coverage.

If you want to review your medical records, call your doctor or other providers directly. You have the right to add to your medical record if the information in your record is incorrect.

All participating providers are required to maintain confidentiality.

Credentialing

Not only are new physicians required to participate in the credentialing process, Paramount recredentials all of our physicians every two years. This process involves verifying state licensure, hospital privileges, board certification and malpractice insurance with primary sources. On-site reviews by staff include an evaluation of the facility, medical records and medical care patterns.
